Ambient Scribing

Ambient scribing built for emergency medicine. palmER captures your encounter and generates your HPI and Physical Exam notes in seconds.

palmER listens during your patient encounter and writes your HPI and Physical Exam notes automatically. Review what it generates, edit if needed, or refine with follow-up instructions.

Use Your Phone

You can use your phone or any mobile device to scribe encounters. Navigate to platform.palm-er.com in your mobile browser where you'll see your patient list. Select an existing patient or add a new one. Tap Start Ambient Scribe to begin capturing.

How It Works

Select or create a patient

Choose an existing patient from the patient panel, or tap + to create a new one.

Start scribing

Tap or click Start Ambient Scribe to begin capturing your encounter. Leave your browser open and your device unlocked while scribing — locking the screen or switching apps can interrupt the scribe.

See the patient

palmER listens in the background while you see your patient.

Stop scribing

Tap Stop when done.

Make additions or clarifications (optional)

After stopping, you can add exam findings, corrections, or details that weren't said aloud — anything you want the AI to include when generating documentation. You can always edit and refine later at your desk.

HPI and Exam auto-generate

Tap Done and your HPI and Physical Exam generate automatically in seconds. Review, edit, and refine as needed.


After Scribing

When you stop the scribe, you have a few options:

  • Supplemental context (optional) — add exam findings, corrections, or details that weren't said aloud. This context is included when the AI generates your documentation.
  • Done — submits the transcript and generates documentation immediately
  • Update & Generate — saves your supplemental context, then generates documentation
  • Discard — discards the encounter entirely. No documentation is generated.
  • View/Edit Transcript — tap the more options menu () on the patient card and select View/Edit Transcript to review or make changes to the transcript at any time

Supplemental Context

Use the supplemental context to add anything that wasn't said aloud — specific exam findings, vital signs you noted, or clarifications or corrections to what was said. The AI incorporates this alongside the transcript when writing your notes.


Auto-Generated Documentation

After you tap Done:

  • HPI and Physical Exam auto-generate from your transcript
  • Review, edit, and refine the generated notes back at your desk

MDM does not auto-generate. You initiate MDM generation when you're ready, typically after your workup is complete and results are back. This gives you control over when the MDM is written and what data it includes.


Multiple Sessions

You can scribe additional encounters for the same patient — re-evaluations, updates, or follow-up conversations.

  • Each session adds to the patient's transcript history
  • The AI uses all accumulated transcripts when generating updated documentation
  • New generations create a new version. You can browse previous versions using the branch picker.

Privacy & Security

No Audio Is Ever Saved

Speech is transcribed and immediately discarded. No audio file is ever created. No one, including us, can listen to any encounter.

  • Patient data auto-deletes within 48 hours. Everything you enter, patient info, transcripts, generated notes, is permanently deleted within 48 hours.
  • Your data is never used for AI training. Your patient encounters and documentation are never used to train AI models.
  • Encrypted in transit and at rest. All data is encrypted when sent between your device and our servers, and while stored.
  • HIPAA-compliant practices. Your data is only used for one purpose: generating your documentation. After that, it's automatically deleted.

Pro Tips

  • Place your phone close to the conversation (within 3-4 feet) for the best transcription quality
  • Minimize background noise when possible — though palmER handles typical ED ambient noise well
  • Use supplemental context — adding exam findings and corrections after scribing can significantly improve documentation quality
  • Keep your screen on while scribing — locking your phone or switching apps can interrupt the scribe.
