Chart View & Chat View
Review and refine AI-generated documentation in Chart View (unified) or Chat View (conversational). Switch between views anytime.
palmER offers two ways to work with your documentation: Chart View shows everything at once, and Chat View offers a conversation thread. Switch between them anytime. Your preference is saved.
Switching Views
Toggle between Chart and Chat using the view switcher in the toolbar at the top of the screen. Your preference is saved and persists across sessions.
Chart View
Chart View displays all three core documentation sections — HPI, Physical Exam, and MDM — as collapsible panels on a single screen. This is the fastest way to review your documentation and copy it to your EMR.
Viewing Documentation
- Three collapsible sections — HPI, Physical Exam, and MDM are all visible at once
- Expand/collapse — click the chevron on any section header to toggle it open or closed
- Status badges — sections that were auto-generated from a recording show an "Auto-generated by Ambient Scribe" badge
Refining Documentation
The input area is fixed at the bottom of the screen and targets one section at a time:
Select a target section
Use the dropdown menu in the input area to choose HPI, Physical Exam, or MDM as the target. Or simply click on the section you're working on.
Type your instruction
Enter your instructions. For example: "Add more detail about the chest pain history" or "Include shared decision-making documentation".
Press Enter
The AI processes your instruction and the updated documentation replaces the current content.
Snippets are available via the quote icon in the input area.
Editing Documentation
- Pencil icon — click the pencil icon on any section to edit the text directly in the browser. Make your changes and click Update.
- Copy icon — click the copy icon to copy a section's text to your clipboard for pasting into your EMR.
- Thumbs up/down — provide feedback on any section's quality.
Version History
Each time the AI generates new documentation for a section, it creates a new version. Navigate between versions using the arrow controls:

- Click the arrows to browse previous versions of a section
- Compare earlier AI output with the current version
- Inline edits (via the pencil icon) modify the current version in place — they don't create a new version
Chat View
Chat View provides a traditional streaming conversation interface.
- Tab bar — switch between HPI, Exam, and MDM assistants using the tabs in the top left navigation bar
- Full conversation history — each patient has a separate conversation thread per assistant
- Same editing and branching features as assistant chat — see Chat Editing & Branching
- Snippets work the same as in assistant chat
