Your patients aren't one-size-fits-all. Their discharge instructions shouldn't be either.

DischargER writes clear, personalized discharge instructions, work notes, school notes, and any other documents your patient needs.

What DischargER Writes

Anything your patient needs to take with them:

  • Sports and return-to-play notes
  • Activity restriction notes
  • Excuse and clearance letters

Written for the Reader

Every document is written for its intended recipient. It uses clear language and the right level of detail, whether it's for the patient, a parent or caregiver, an employer, or a school.

For third-party notes, DischargER includes the minimum necessary information. It will not include diagnoses, test results, medications, or other clinical details unless you specifically ask for them to be included.

Where to Find It

    Assistant workflow

    Open DischargER under Tools, paste the chart, and ask for the document you need.

    Patient workflow

    Open the right sidebar and select DischargER. The chart is already loaded, so just ask for the document you need.

One-Tap Suggestions

Tap a suggestion or enter your own instructions. You can create any document your patient needs.

What DischargER Includes

DischargER creates patient-specific discharge instructions, including home care guidance, follow-up recommendations, and return precautions. It also explains important results, pending tests, and incidental findings that need follow-up in clear language for the patient.

Your Documentation Preferences

Your custom instructions for patient-facing documents live in Documentation Preferences → Discharge, under your user menu. Set them once, and every DischargER document follows them.

For example:

- Write discharge instructions at a 5th grade reading level.
- Refer to our hospital as "Memorial General Emergency Department".

Refining a Document

You can edit and refine a document without starting over. Ask DischargER to make the instructions shorter, simplify the language, translate them, add a specific restriction, or create another document for the same patient.
